LSFP Accepts Most Major Insurance Plans

Patients are responsible for co-payments and any additional charges not covered by the carrier. 

As an additional service, we will process insurance forms.  The practice does not, however, assume the responsibility for collecting insurance payments or for negotiating settlements for disputed claims.

For non-participating commercial insurances, our office does not accept insurance coverage as a substitute for payment.  Patients are provided with a "superbill" with which to file their claims.
